*SCHOOL PHYSICALS FOR 2024-25 SCHOOL YEAR
Students planning to participate in any form of competitive athletics during the 2021-22 school year must have the health certificate of the KANSAS ACTIVITIES ASSOCIATION completed and on file in the school office before the first practice of that sport. This form must be signed by a parent or guardian and by a physician. Students who will be enrolled in physical education must also have the physical form on file before class begins. Parents need to complete the Health History portion of the physical form and make sure that both the parent and student signatures are on the form.
